On the Atari ST the entire Midi stuff is handled by a dedicated serial chip, you can basically read the bytes directly from a small FIFO that can store up to 6 values I seem to remember, enough to handle an IRQ, fetch the data, and continue doing the work.

IIRC Fabrice (Euphoric) showed a Telestrat playing Midi files to an expander at a CEO meeting, think it was arund 2005.
I guess the Telestrat was modified but I can't recall (I missed the explanations).
